Hello, that would cause the temp gauge to rise possible all the way, depending on how much and how fast your losing coolant.

How do I check to see if the radiator fan is working? How do I check to see if the fan belt is ok? If the fan is not working will it cause the water in the radiator to evaporate or does that only happen...

You can raise the hood and let the car run untill the water temp gets hot and the fan should come on and the temp hand riseing after getting off freeway could be that at faster speeds you have more air going thru the radiator so make shore the radiator dosent have any trash stuck to the front of it . And it is electric fans so their wont be a belt and if you are having to add water often it sounds like you may have a small leak somwhere.Hope this helps.

it sounds like you are running out of coolant. refill and check for leaks. The coolant reservoir may have a leak. Your thermostat may need replacement.

1999 accord is leaking small amount of radiator

If a coolant temp sensor is not working, the fans will not activate, and fluid will go overboard the reservoir, and the temp gauge should have an abnormal reading. Happened to my 1998 accord that the top plastic tank above the radiator had a small leak right where it meets the aluminum honeycomb. It leaked only when hot, and evaporated right away, leaving no traces. Start the car and let it idle for 5 min, and then open the hood and check if the fans are coming on, and if you can see bubbling coming from the radiator top assembly.

Car is running hot (temp gauge rises above normal)

Try the overflow tank cap. The seal will go bad and not hold the psi causing the water to flow out this is something you will not always see which will cause the engine to overheat or temp gauge to rise. Also the thermostat could be sticking and cause the temp gauge to rise. Are the fans turning on when the temp gets high? If so then I would resort to the above first. If the fans are not coming on, the relay or fan/s could be the problem.

you have 4 water lines, they are 2 radiator hoses (top and bottom) the bottom one runs from the lower part of the radiator to the water pump, the top radiator hose runs from the upper part of the radiator to the thermostat housing

2 heater hoses, they go from the water pump to the firewall.

if you know which one has a hole then you should take the van to a parts store (if you can safely drive it that far) and show them, they can get the right one for you... and sometimes you can talk someone into putting it on for you.

as for the fluctuating gauges you probably need new sending units for the temp gauge and fuel gauge

I heard a squiky noise like some kinda old belt rotating, then it stop. 2 miles later i saw temp rise. i pulled over and antifreeze was splatter from the top of radiator.

Check the radiator hoses for a leak. The splatter was most likely the coolant/anti-freeze leaking from one of the hoses. Loss of coolant would make the temp rise and the engine overheat. If the hoses are good, look for seepage around the waterpump. These are the main causes for coolant loss.

Intermitant temperature

Check to make sure your thermostat works right by monitoring the warming up of your engine. With a completely cold engine, start and run the engine for a few minutes, then put your hand on the top radiator hose to monitor if and when it gets warm as the thermostat opens more and more and note the gauge reading( if it works) the top radiator hose should be unbearably hot within 5-8 minutes. If the therm. opens right it should close right aswell, staying open a little while eventhough the radiator may have cooled may be what you are experiencing causing the rapid cooling and is nothing to worry about. The Hose should not get warm very soon after starting the engine. Regarding you gauge, lets take one step at a time, verify proper op. or the therm.
