Well thats a good question. the sensor may be ok and your engine is running lean, or the sensor is bad............its possible that its bad, but I would want it checked by a qualified repair shop first, you dont want to replace parts because you THINK they are bad, that just costs you even more money. The upstream is located in the bottom of the exhaust manifold. if its lean found out why..... vac leak?

Position of oxygen sensors

all you have to do is follow the exhaust from the manifold back and you will come to the sensors. bank 1 O2 sensor is on the bank cylinder 1 is and bank 2 O2 sensor is located on the bank cylinder 2 is on. the first sensors you come across is sensor 1 then the next sensor you come across sensor 2 and so on. so you can have a sensor code like bank 1 sensor 2 meaning the sensor on bank one but the sensor is the 2nd sensor in the exhaust.

Have you checked fuel pressure? Random misfires (0300) are usually related to fuel, spark, or compression issues-one of those is not right.

My 2002 ford e-150 is pulling p1131 code and p1151 code what is the problem

Lack of upstream heated O2 sensor bank 1 & bank 2. I would first check for broken or corroded wires to both upstream O2 sensors. If no apparent bad wires O2 sensors are bad.

2005 gallant with O2 sensor engine light is one...how do I fix it?

have the engine light code scanned, and it'll tell you if it's bank 1 sensor 1, bank 1 sensor 2, bank 2 sensor 1, bank 2 sensor 4.

Bank 1 is on the same side as Cylinder 1
Bank 2 is on the opposite side of Cylinder 1

Sensor 1 is located before the catalytic converter.
Sensor 2 is located after the catalytic converter

Spinning My Wheels!! ---- P0421 Code Mitsubishi Galant

I have the same vehicle, same code P0421, changed the BOTH O2 sensors and the check engine light was still on, took it to my fathers garage and checked the Catalytic Converter, and it was ok later I found out that P0421 means that the vehicle doesn't have the proper mixture of gas and air (running richer) so I changed the MAX air flow sensor and wala the code went away.

CATALYTIC CONVERTOR - BANK 2

You have a V6 engine, bank one is the row of cylinders on the passenger (right) side, bank two is the row of cylinders on the drivers side (left). Your codes indicate an inefficiency in the cat on the left side but this could be due to a bad O2 sensor or an engine that is out of tune and overloading the cat. This car seems too new to have a bad cat. I would change the plugs and the O2 sensor after the cat before going any further. The reason I say the O2 after the cat, it's the one that is critical in sensing cat efficiency. What is the mileage?
