Ac and heater wouldn't operate on #5 setting for a while then now blower stopped all together

Most likely cause is a blower motor resister, you will need to know if it is a 2 or 4 hole blower moter resister. If that don't fix the problem then change out the blower motor.

2004 Envoy Blower moter was working on all settings except #4. Now the blower will work only once in awhile at any setting. Could this be a blower resistor problem? If so how would I change this out? Do I...

It probably is the resistor. The connector on these also would melt. If that happened, you have to replaced the blower resisitor and the connector. The replacement connector is available at dealers, but all the leads are white. It's best top replace each wire one at a time, so you don't cross any wires.
