Any engine will have trouble ticking over if only 3 of the 4 cylinders are working. Check the spark plug for a consistent spark on all cylinders. Check the connections to the fuel injectors on the cylinder head, and check the compression in all cylinders (take spark plugs out and use a compression tester whilst turning the engine over on the key). Low compression in one cylinder may mean head gasket failure, valves out of adjustment (staying open) or bigtime piston or piston-rings failure.

Do a tune-up or clean your spark plugs.

Answer to question 1) Absolutely, what you are describing is exactly what you would notice and feel with a faulty c/o! Significant power loss and a shaking/vibration that feels as though it's coming from the transmission. I would venture a guess that the computer detected an engine misfire before you noticed the problem and it gave you a "heads-up" with the check engine light. I would go ahead and change all of the plugs if it's been a while and at least the c/o on these 2 specific cylinders.
Answer to question 2) Several things could cause this sound. First, and most likely with the 5.4, from what you describe you have lifter "ticking". Typical for Ford, not hating, just saying. In fact I own a 98' like yours. The other possibility is that it's an exhaust leak. You can determine which it is with a mechanics stethascope or even a short length of garden hose. With the vehicle running, take your instrument and place it very closely at one end of your valve cover and the other end to your ear and slowly move to the other end of the cover listening for the "tick". Now move to the other cover and repeat the process. If in fact this is where your sound is coming from it will be very obvious when you find it. If there is no noticeable change in the sounds at the covers then begin to inspect the exhaust system starting at the manifold coming off of the engine. Hope this info helps to solve your problem!
