I have a 2000 GMC 6.0 with 165.000 miles. This stared about a week ago and seems like it has gotten louder since, but just on startup. On cold starts or after it sets for an hour or better it sounds like a lifter is not pumped up, if driving with windows up kind of sounds like an exhaust leak. After engine has run for better than 10 or 15 minutes noise finally quietens down but not completely. Oil pressure seems to stay around 45 to 55. From what I understand this engine has the hydraulic roller lifters. Any ideas whats up??? Also if it is the lifters, can the be changed with out pulling the heads? Where and what brand would you recommend purchasing? Thanks fo any info.
Try a cup of tranny fluid, Dextron, or whatever, in the crankcase. Or there are lots of lifter products at your local part store. I would certainly try that first. I assume your oil has been changed recently, and the level is good. You don't have to pull the heads, but you do have to remove the intake, and valve covers.

a temporary "fix" for this issue is just to use a heavier grade oil. if that doesn't quiet it down you know perhaps that it isn't the lifters.
