It maybe in the fuel tank and not serviceable check with the local auto parts im assuming you have traced the fuel line from front to back

Toyota 4 runner fuel pressure
pressuer should actually raise under throttle, but not much. maybe 5psi. 38-44psi is the correct range. if your dropping much I would replace the fuel filter. next I would check the tank for debris or water, finally i would replace the Fuel pressure regulator.
