2005 Dodge ram 3.8 manual, I've changed the throw out bearing and clutch. Still making noise, want shift in gear while running. What could it be?

Did you change the pilot bearing when you changed the clutch? The pilot bearing is in the center of the flywheel, and the input shaft of the transmission fits into it. If the bearing is frozen, or nearly so, it will cause the input shaft to spin with the flywheel regardless of whether the clutch is released or not. I had this issue on my Dakota, and I had to turn the engine off to get the truck in reverse or first. Once going, the synchronizers allowed me to shift up.
If you can start the engine while in first gear and the clutch depressed and your foot on the brake, then slowly release the clutch and the truck moves, this is probably the issue.
Unfortunately, to replace the pilot bearing, you have to remove the transmission again.

the procedure is same on all 4wd cars/trucks(mostly)
some pull the engine , for sure if engine needs work,as most do
after 25 years,
or pull transfer case and trans,pulled at the bell.
depends too,if you like spending long time under car
or on top, bent over fenders. (and tools)
if engine out no need to drain 1 or 2 gear boxes..and links,console..endless details....4wd lots more....

the video above shows the odd DUCK, throw out bearing.
and is important to know all that,
why not get a service manual..>?or use mitchell's????

done many jeeps and rice 4wd.
2.6L,, SACHs 9.5" clutch $120 ($300 is bs)
or
2.8 v6 SACHs 9.125" clutch. $170

see they are not the same.....
I like Sachs, as you can see.. china clones not.

do you know how to fit the flywheel pilot bushing
and inspect fly surface, ?

or you wiggle said shaft and front bearing in the box is SHOT.?

just saying, ive seen it all there, every fail.

You experienced a bearing failure, do you mean the pilot bearing or the throw out bearing. Very odd to have pilot bearing go, unless very high mileage and was not changed when clutch was done.
Dodge diesel truck forums talk of problems with pilot bearings, not so much about Dodge gas.

Rod bearings are certainly a cause of knocking sound in an engine. You drive it hard? Don't change the oil, or let the oil run low? Have a whole lot of miles on it?

Main bearing cap bolts there is a outer cap bolt that are torqued to 20 ft lbs + 1/4 turn. Then there a inner cap bolt that are torqued to 15 ft lbs + 1/4 turn. Con rod bolts are torqued 20 ft lbs +1/4 turn

I found this web reference for a 2 year older Dodge that might help:
On 98 dodge dakota with 3 9 V6 how do set up the distributor
This web reference indicates a VAST difference between the V6 and V8...so you could be on the right track with your 180 degree comment:
"The V8 mark is 17.5°after TDC and the V6 mark is 147° after TDC"

1991 Dodge Dakota

remove tansmission and clutch and pressure plate just pull it out and 1 trick is to take a grease gun an stick it through the hole in the pilot bearing and shoot grease and it normally will force it out of the crankshaft
