Washer Fluid is almost always supplied from the same reservoir as the front windshield/headlamps for American vehicles, the distribution system is split by using 2 or more pumps at the discharge of the reservoir. There are a few European autos that utilize 2 reservoirs but these reservoirs are usually right there next to the main one.

Hi I have a Toyota Hiace 2006 model and can't seem

The rear washer and the front share the same water bottle in the front, if the rear only is not working the hose to the rear may be disconnected or the motor for the rear washers is bad, there are two motors on the bottle one for front one for the rear, make sure both are working.

On my 06 equinox the rear window fluid is empty and I cant find how to refil it

The fluid for the rear washer just comes from the front fluid container. If it is not coming out through your rear squirter, try and hold the lever that makes it spray for about 10 seconds. If it is still not working, there may be either a blockage in the lines (if you live in a cold climate it could be frozen), or the line may be broken or disconnected, or the pump that supplies washer fluid to the rear window could be burned out, or the fuse could be blown.

If you activate the rear window washer, do you hear any pumps making any noises? If it is making noises, then the pump works, meaning either the line is frozen, broken, or disconnected.
If you do not hear pump noises, then it is either your fuse for the pump, the pump itself, or perhaps even the switch (which is less likely)

All of these solutions are only going to help if you make sure that the washer fluid container is full :)
