How hot does it get in degrees F? Does the radiator fan work? Some makes and models, the electric fan won't come on till about 220 degrees F.
You could have gasket issue? I saw what you replaced, but, did you check coolant flow through the radiator, cool engine. Sometimes for testing, you could leave thermostat out, hook everything up, make sure full of coolant, the coolant flow is instant, when you start it, You don't have to wait for it to warm up.
Also.
Put the car up in the air high enough to get underneath it.
With the motor cold and the engine not running, feel the bottom radiator hose from end to end.
You should feel a spring inside it.
If that spring rusts in two, or slips to not be supporting the whole hose, the radiator hose can collapse together when you are driving down the road, usually at over 20 - 30 MPH.
You would then need a new hose.

Since your blower is working and it is a heat issue,thats where we'll
start,First thing is have you checked the coolant level in the radiator when
engine is cold,make sure it is filled to top,next verify you have coolant in
reserve ,Start your van and get it up to normal operation temperature,If your
heat gauge on the dash works correctly it should be 190 degrees,Now lift the
hood and locate the two heater hoses going into the heater assembly at the fire
wall,Both hoses should be hot to hold on to,if both are hot and no heat inside
your problem will be the blend air door in heater assembly.
if neither hose
is that hot you need to replace the thermostat in the engine to get it up to 195
degrees.
if one hose is hot and one is luke warm then the heater core is
blocked and to blame. hope this helps you.

If the heater core is not damaged or leaking ie: if you don't see coolant on the INSIDE of your windshield or coolant on the floor inside the car then chances are you may just need to BLEED the radiator.
remove radiator cap
Fill coolant resovoir to the cold line if the engine is cold from sitting overnight.
Start engine with cap off
Wait till engine gets hot
While waiting you will see a big gurgle of air bubbles come out and the coolant level will drop
Fill the radiator with more coolant to compensate.
When the fans come on and it stops gurgling air install the radiator cap
You are done
Watch the coolant level for the next few days and top off in the resovoir as needed to keep it filled.
Your heat should be very hot now providing you installed the correct oem thermostat correctly.
