The O2 heater receives power from the PCM. Did you check the O2heater ground connection?

bank 1 refers to the side of the engine that #1 cylinder is on , and sensor 2 refers to downstream , after the catalytic converter. , this link shows bank 1 on a 2001 nissan maxima V6 being on the firewall side . if you had a 4 cylinder it would be downstream after the catalytic converter .

Bank 2 will the the passenger side or right side and sensor 2 is the O2 sensor behind the catalytic converter.

Bank 1 is the frt side exhaust manifold (did they say bank 1 sensor 1?),you can start by replacing that sensor,i would check the intake for leaks ,the gm 6cly. are known for bad intake gaskets,you will see oil coming out form the frt or rear of the intake,also check your atifreeze overflow bottle for bronwish sludge,which means that the oil is mixing with the antifreeze.A leaking intake could cause that lean condition.

check the mass air flow sensor there maybe dirt on the filiment inside the sensor causing false readings. cleaning or replacing it may make the codes go away.

Yes you have a problem with wiring for the O2 sensor shorting out somewhere in the line. It could be anywhere from the sensor to the ECU. Make sure there are no breaks, burns, or crushed parts of the wiring.

Usually the code means the O2 sensor is faulty.
But in your case it could mean there is a problem with the fuel system on bank two. Bank two is the side with cylinder two.
A low voltage reading from the sensor would mean that side is very lean if the sensor is working and the sensor in front of the catalytic converter shows the same readings.

Locate o2 sensor bank 2 sensor 2

Depending on what type exhaust you have ( dual or single) you will have several oxygen sensors. The bank # XXXXX means it is one of the o2 sensors on drivers side. The sensors are located screwed directly into the exhaust pipe at different spots. See below. You will have say sensor 1/1 which means sensor 1 / bank 1 etc..
