2007 420 Rancher ES. Why is it stuck in 3rd gear and seems to be flashing a code of 12?

Most transmissions go into a fail safe mode to prevent damage to transmission but you can still damage it if you keep driving it. Most related codes are due to speed sensors. But you will have to look up what the code means exactly.

What does the P stand for on code PO420P?

The "P" stands for "Powertrain Codes". ( Example: PO101 - Mass or Volume Air Flow Circuit Range/Performance Problem.) "B" is for Body and "C" is for Chassis.

When idle it will shake and when takeing off it takes off it hesitates

you probably need to tuen your idle up a bit cause it's down to low, which causes ur car to shake or vibrate when your at a stand still.

o/d stands for overdrive. most cars with automatic transmissions that have overdrive have a button ussually on the shifter not sure on the lexus where it is but try to find an overdrive on off button

pretty easy job.
you will need basic tools jack,stand

jack car up from jacking point d/s near front mud flap area you will see what looks like jacking point or check manual
place jack stand under A arm d/s axial or any safe point see manual
remove lug nuts
remove tire
behind brake caliper is 1 sm bolt think it 14mm need to remove
loft up caliper it will pivot on the other bolt
remove old pads
followinscructions for new pads add brake quiet to new pads and shims reinsert
reverse order. thats it
