The shop quote could be correct about insufficient fuel (lean)
condition. If this exists it causes the driver to push harder on the
accelerator (more fuel) to "make up" for the lean condition thereby
causing your truck to use more fuel and lose gas mileage.
I recommend looking for a vacuum leak at the carb base, intake manifold, vacuum lines or the power brake booster. Try to use (borrow) an external vacuum gauge during testing. It should read between 17-20 inches of mercury during normal idle.
Has the Toy had the flat spot as long as you have owned\driven it? If so the carb might need to be re-jetted especially if it is being used at a different altitude then what it was set-up for.
