Does the clutch pedal go to the floor? If not the clutch is not the problem and the transmission or transmission linkage is, inspect all components for loose or damaged components.

when you shift into 5th gear take it real slow and easy on it this is a result of the syncranizer in fifth gear going out if you take it gentle this will last a while without having to replace it hope this helps you have a great day
About all you can do at this point is change the oil in the gear box and have your shift linkage checked and adjusted. The grind is internal, and the transmission will need to be removed to be repaired. Most likely it's worn synchronizers.
You probably have air in the system. You need to bleed the air out of the system so that it will disengage your clutch. The air in the system is compressing and, therefore, the slave cylinder can't reach the full extension.
