Replace the clutch package. You have to pull the transmission Master and slave also..

My 91 4Runner has problems sometimes going into reverse,either it just wont go in without an awful grinding or its just difficult to get into gear. also as I'm driving down the road in second and...

had simular problem was ready to tear out the cluch and throwout bearing when just by chance i decided to check pedal clearance measurment. what i found was the brake/clutch pedal assembly that mounts to the fire wall had a small split in the metal right beside the clutch spring from yrs of being pushed to the floor and evry time i pushed the clutch it only go so far b4 the metal would start flexing so it wasn't ingauging and all the way.fixed with running a hole all the way through firewall by the split and bolt&nut tighten down.worked way better then i would of thought. or if no defects in metal you can try letting out all the way on the clutch pedal(with it out of gear) after you start it and pumpimg it one time so the bearing gets a chance to stop and get where its supposed to then try putting in reverse and see if it helps(know that this is a temperary,cheap fix to a permanent problem untill parts can be bought)

Usually reverse does not have synchroniser , so it's normal if the first gear is going in ok.
If you have problem to shift into first gear , then it might be a problem with clutch or clutch hydraulics.

First you have to bleed air from clutch fluid.
if it is not ok, Remove clutch agan & check pressure plate finger blance or damage.

91 toyota 4runner 3.0L has very rouh idle, hard

All "TUNE UP' Components MUST Be Eliminated as suspect to go further with Diagnosis. They Should be 1. KNOWN to be or CLOSE to NEW or 2. Recently Replaced.
What you have is Obviously a Engine Performance Related Problem, Thus These Items Must be in very good condition when checking for a Problem here

'00 Toyota 4Runner manual transmission slipping out of first gear

The problem is most likely internal. Sounds like it could be the synchronizer or maybe the gear itself. Hard to say until you take it apart and look at it. I guess a price range of around $1000-$1500 to rebuild, repair costs depend on what parts needed, cost of parts, labor/time involved.

check the clutch plate fingers for bends and the connecting forks for misalignment or off-center bolts, especially if it was a DIY.
