Hi Franklin:
All of the engines I've played with that had hydraulic lifters also had hollow push rods. The lifters act like little pumps and oil comes up through the push rods to lubricate the end of the rocker arms.
My answer:
flat end down onto the cam
hole end up for the push rod to sit in.

Two items that are important here.....Did you check the rocker arms for wear and did you check the camshaft to make sure the lobes aren't worn? Also, even a new lifter will not function correctly if the oil pressure in the engine is low. An engine with 200k on it likely has some bearing wear. The oil pump can also wear but generally it won't wear enough to lower the pressure enough to cause lifter noise. If everything in the valvetrain checks out as good, I'd be looking at the bearings. If some are worn (check with plastigage, available at parts stores) replace them and also the pump.
Most times, the rod bearings wear faster than do the mains. You can do rod bearings in the car. If mains are worn, best to do a complete rebuild as cam bearings are likely close behind, and mains should be done out of the car anyhow. (doubt if you will need to go that far though).

What exactly are lifters?


lifters ride on loped surface of the cam shaft the other end of the lifter a push rod sits on , when the cam shaft rotates the lifter moves up and down allowing the push rod to move the rocker arm which opens and closes the valves.

hold up rod bearings were did you get that?lets take it from the top.camshaft lobs move lifters on rotation,which lifters move push rods,push rods push valves up and down,which other side of valves push up and down valve stems,which open and close exhaust and intake stems.ok now were clear on that to see your push rods.take off your valve covers on side with tick.1.check for valve loosness.need a feeler gauge for this and specs for you truck to do this.because ticking is mostly associated with valve loosness or to tight.2.if rod is bad loosin valves but not off and inspect the rods for bends.on a flat surface.replace if bend.also make sure they to go back in same place.and repeat step 1.finally noise continues need to take in and checked possible internal damage.

Push rod replacement?The push rod seat is missing and the push rod just falls through. What should I do?


Where did it go?? Just wore through? You have to get the lifter out of there and replace it with a new one, or a used one. For some reason, it is not getting oil. You might be finding more damage as you go. One of these days you are going to have to fix that engine right, new cam, lifters, etc. You can make a hook out of 1/4 inch rod, grind a flat spot in it with a small shelf left at the bottom for the hook, it will grab the edge of the lifter and pull it out. Hope this helps.

Replace lifter


Lifters are usually replaced in sets with the cam or rods and rockers, replacing just one lifter, rare, but if it is making noise, then just locate the trouble maker, you can run the engine with the rocker cover off, find the bad one, shut of engine, loosen up the rocker, twist it out of the way, pull the rod out, then go in the hole with a piece of heavy wire, rod, steel, with a cut in the bottom to hook on the lip of the lifter, then pull it out. Put it all back, you should be all set.

Most likely its just your lifters clapping as they make contact, until the oil starts flowing good through them, then it goes away. My car has been doing that for a long time. Never had any problems. The rods are pushed up by the camshaft, the rods push down rocker arms which then open the valves. The rods are round at the tips and sit in lifters that pump oil through them up to the top of the engine and then out onto the valve springs. If your car has a lot of miles you may consider putting a thicker oil, something like 10w40.
