The transmission has internal failure. Clutch packs possibly worn out, shift solenoids failed, you need a transmission guy. And from the sound of this, probably a transmission.

Has the 'Check Engine' light come on? I had this problem at 30,000 miles and the trouble codes set were transmission component slipping. Ultimately required replacement of the transmission.

Code 81 is brake switch input circuit.The being stuck in third is a limp mode because of a hard code being set.If you repair or replace the brake switch issue you should be fine on the shifting issue.Good luck and let me know if i can help.

it could be the shift selecter on the side of the tans. the4l60e are bad about this problem ajust and make sure it tight.
