You have a bunch of testing to do...start with fuel and spark

On top of the air filter cover there is a valve. I think it is called an mass air flow sensor. Try tapping it lightly while idiling is bad. And see if it starts idiling better (gets unstuck). This could be the problem. ( to much or to little air) .

A diagnostic scan is always a good place to start, even if there is no check engine light on. Ignition...are you getting fire to the spark plugs? Fuel pressure, fuel pump, clogged fuel filter, air filter not restricted? Battery fully charged, terminals clean and tight? These are a few basics needed for proper engine mangement.
Good luck...hope this helps...please rate me a fixya.

1991 Toyota Pickup 3.0 idle problems

I would clean the idle speed control valve (IAC) and the throttle valve and bore with CRC brand throttle valve cleaner, use about 1/2 of the can, hold the idle up so it won't die while you spray it into the throttle valve. After you are done disconnect the battery negative (-) cable for 5 min's and then drive it for a few days so the system can relearn the idle strategy for your driving habits

Idle surges and dies

The first thing to look for is vacuum leaks, possibly in the EGR valve or its related components, also look at the heat activated vacuum switches on the intake manifold,an easy way to check is to spray a small amount of wd-40 around the vacuum connections if theres a leak it will pull it in and can the motor rev up.I just worked on a Toyota that was doing the same thing and it was a loose EGR connection,Hopefully this helps.
