I suggest you have a transmission specialist look at it. The jerk could be the transmission 'slipping'. A wrench light usually means a service is needed. The specialist will be able to service the transmission and find any other issues.
we cant drive car, so jerking is not clear to a non driver.

and that means jerking can be 2 things (sure more but)
engine jerks, does it only?

get it serviced, with persons that can drive and experience this live.
Put a line pressure gauge ($55 shipped on Ebay, or visit a local trans
shop) on the pressure test port and go for a drive to verify proper line
pressure. Base pressure at idle in P,N,OD should be around 70psi and
around 140psi in R. Max line pressure should be at least 190psi at WOT
in 1st or 2nd, 225psi is optimum. If base pressure is low, trans or pump
is worn out. If max pressure is low, check EPC solenoid. If reverse is
slow to engage, and line pressure does not come up to at least 130psi in
reverse, replace the boost valve with a Transgo 0.490" boost valve
(easy fix in pan).
Slow, slipping or no reverse: "lo-reverse" clutches are worn out,
fluid leak in the reverse apply circuit, or broken sunshell. It is
possible to remedy a fluid problem by removing the checkball from its
cage in the case in the rear of the trans may help (must remove
valvebody), or adding a high-viscosity additive such as Lucas
Transmission additive or other seal restorer product. May also have worn
boost valve (can replace in the pan)
From what you have described, it may just be a broken Sunshell which is the weak point to the 4L60E transmission. Gm had a upgraded program to lower the transmission line pressure when revers is engaged to keep the Sunshell from breaking when put in to revers.
The revers band sounds like it needs to be adjusted so it's not slipping. You caravan is a 2008 so the transmission should still be under warranty unless you do lots of driving around, if it's past the warranty coverage, bring it to a transmission shop and have the fluid, filter replaced and the revers band adjusted since the transmission pan is going to be dropped. Good luck friend. Keep me posted if you want to try to tighten up the bands your self, nut i think it's better for you to take it in, Less mess and job will be done right with warranty.
you should go to a transmission guy they have computers that will find the problem and i think that its free... but i think that your transmission needs to be rebuilt, theres a lot that can go wrong with them things

Are you getting the check engine light? If so the electronics may have gone out. You may not need a rebuild but you won't know till you pull the oil pan on the transmission and inspect the contents. You may also have lost the neutral safety switch. It is mounted on the drivers side of the transmission. Check those out first and let me know what codes you get.
For Ponchoboy; the clunking and hissing you hear are teminal root causes. Drop the oil pan and if it has a lot of filings and metal it is time for a rebuild.
