Not all the time but when I start the car it starts very hard. When it starts hard like that and I wait a few second and put it into gear it slams into gear and sometimes won't move.

hard starts are usually never getting a tuneup, (like spark plugs?)
slamming in to gear is normal if RPM is too high in park, is it.
wont move , in any gear, car wont move even in reverse.
and RPM of engine does what , sticks low, like 800 rpm
or screams at the advancement of right foot.
need lots of details. we cant drive car, but if could in 5min flat all that is clear,
eg:
transmission slips
engine weak
engine racing in park too high.
wont start and misfires when it does.
is fuel fresh, not year old.
ever get a new air filter and fuel filter. if filter is outside tank.

My car will go off gear when i brake hard and it takes a few seconds to pick up gear again

It sounds to me like your transmission fluid level is low, when you brake hard the fluid moves to the front of the pan and leaves a void for the filter to grab air instead of fluid. When you stop the fluid settles back and away you go again. This could cause premature transmission failure so check it soon and top it up ASAP. With the transmission cold the fluid level should be near the add mark, full mark only if the trans is hot.

0.transmission takes a minute or 2 before reversr gear engages

The anti-drain-back valve in your torque converter has gone bad. The torque converter drains into the tranny when you shut down, and you have to wait until the pump refills it before it can work.

The pump does NOT fill the converter in park, only in one of the gear positions, or neutral. Start it up in NEUTRAL, wait a minute, and drive off.

Transmission

make sure your yoke nut on the rear end is tight if you have slop in that it will cause it to slam into gear. I had a 94 f 150 it happen to.
