Is there any trouble codes being set yes/no 1thing that could be possible here is that you have activated your cruise control if you have it by your hand position on the wheel it's happened to me and has been responsible for run away vehicles because people don't realize there hands are on the controls if you do have cruise control try to put your hands in a different position for a while and see what happens you might find that this is it they turn on very easy at the slightest touch and all you need to do is rest your hand on the set accelerator button and it's off to the races/////// if not try cleaning all your emissions connectors and using carbon conductive grease and a tooth pick just a dot will do it's possible that from oxidation/corrosion after 26 years your truck came out in 89 that the power surge is from the voltage transfer being weak then it hits and increases & so does your rpm's because your computer works by voltage signal/voltage pulses this is very possible try these 2 things and post back with results anyway doing the grease on your connectors is an electrical system tune-up that is needed and you'll be glad you did it better mpg's and 100% current transfer like a brand new vehicle you'll feel it instantly in your gas pedal it will get very firm and require less pedal travel for speed the grease can be used everywhere from battery terminals to seat switches every electrical contact/plugs in the vehicle will benefit from it
Run the fault codes as it may be a problem HO2s or O2 sensor.

fuel pressure regulator on the fuel rail is probably it or the egr valve is stuck open allowing burnt fuel to flood the intake, im leaning towards the fuel pressure regulator because the o2sensor is probably reading rich or if its reading lean then i would choose the egr valve . good luck

It sounds like you have a intake leak or vac leak . The codes to lean are caused by air that is getting in your engine that is not being measured by the computers sensors

Sound like you have a big vacuum leak. Check the hoses that attch the PCV valve on the valve cover on the passenger side. Also reach way back on top of the engine there's another hose there that is known for going bad. The dealer has those hoses and they call it the Heated PCV valve hose assembly. Very common problem.

check timing make sure it;s ok . second check your exhaust system if it;s close , start accilerate for 3 min. then look all exhaust pipe (silencer) if there is difrent coler . check Oxgen sensor it;s ok .
and if you have no enough fuel presure try close the return line temporary . maf, sensor only when there it is highand low accilerator.
