Is the trans oil in good condition and correct level ? i'd start with checking that , then if not any better get the OBD codes read as may be a sensor/switch etc

It could be lash in the differential which is normal, but it could also be a loose u-joint in the driveshaft. Check the rear driveshaft u-joints.

You can usually tell by grabbing the driveshaft & turning it by hand back & forth & push up & down, side to side looking for play in the joint.

I have a Focus that had shifting problems. I tried different fluids, but eventually I tried Royal Purple Synchromax (I believe) and it actually shifts fairly well now. I used to hate the car for this. It has 130k miles and I like driving it now.
