This could be caused by a few items. If this is an old car I would suggest that you have the freon checked if they are at optimum level. Freon may be low that only the from part is working. Check also for leaks. AC shops would use a dye to detect leaks in the system.
If your car also works as a heater check if the heater part works. If it does work it may be the blend door that is not working. The blend door has motors and linkages which may have gone bad. A visual inspection is needed for this. If the heater does not work you may have to go back and check for leaks or wiring issues to the rear AC (also for those without heating).
