Not all vehicles have grease fittings. Some are sealed but if it absolutely needs grease then put a needle nozzle on your grease gun and put pin hole into grease boot on ball joint, tie rod,etc. and give it some grease.

Limited information in the link. Really you need a workshop manual and then refer to the section that you might want to lube. Anything that moves like hinges, you just give them a squirt of oil. Engine oil and filter are the main lube concern. Things like steering joints have grease n1pples that you connect a grease gun to. I don't know of anything that is referred to as a 'lubing port'.

grease fittings really dont go bad unless something hit or damages them

We took our car in we bought a tire and had it mounted we also had the oil changed and a lube job.

Modern cars don't really have grease fittings anymore.
If the car was an older car it might have had some.

If you mean "grease fittings", none. By 1996 most cars were "permanently greased".

I hear a sweaking noise when i brake or when i step on the gas coming from the transmission or rear end area. What could this be?

If the squeaking changes with the speed of the truck you need to get the U joints check. At least 1 is dry of grease and will have to be replaced. Hope this helps.

Where are the grease fittings ?

Manufacturers generally removed grease fitting from suspension part back in the 80's. Some put a removable plug in the ball joints, tie rod ends, etc, which could be removed and grease fittings installed in their place, but most have sealed and permanently greased. In other words you can not lubricate any fittings. If parts go bad, the replacement suspension parts generally have grease fittings.

Blown motor in 93 Toyota 4x4, want to replace with 98 Tocoma motor. Any issues with swap?

by all rights it should be a great fit.
but all things look good on paper!! Seriously....your swap will be fine,as the years are fully interchangable. you may run into some wiring issues with sensors,and that can be fixed by just splicing in new connectors that fit them.

Dry squeaky grinding noise 2003 explorer

That's where I'd go first. check with driveline unloaded (neutral with wheels off the ground.)
