The chain has tension adjusters you might have missed this to release and reset them also did you have the crankshaft and camshafts held in place while installing also the chain guide/s could be worn most have plastic guides on the guide rails and contact could have broken this plastic resulting in chain to metal contact I don't know what kind of car your talking about but if its a dual overhead cam with 2 chains this could be it you must you special cam and crankshaft positioning tools to make sure that its in the exact position also your crankshaft sensor or camshaft sensor could be unplugged or not working and can not tell the computer where it should be resulting in a loose chain LATER MAN!

They open the engine and replace the timing belt tensioner, but that was not the problem

Valves Yeah. Put Some Valve Cleaner In Your Gas You Can Buy It At Pretty Much Any Auto Parts Store For About $5.00 If This Doesnt Fix It You May Need A Valve Adjustment.
A Valve Adjustment If Done Yourself Will Take A Repair Manual, Feeler Gauges And A Wrench.
The Timing Chain Is Usually A Steady Scrapping Sound.

need to have a shop fix it asap.if the chain breaks or skips timing your entire could possibly need to be replaced.replacing a serpentine belt tensioner is fairly cheap and easy

Either the camshaft are seriously out of time or the piston rings/pistons are worn out.
Check TDC of Piston No1 on the power stroke coincides with both inlet and exhaust valves being closed.

at the dealer we rec them at 100k,,,,even if its not noisy or anything,it is a constant wear and tear item and the chain is no bigger or stronger than a bicycke chain,,,,they stretch and can break,and if the tensioner breaks or releases tension,its as bad as the chain breaking,either way the pistons will hit the valves and bend the valves,,,,also the timing chain guides are plastic and wear out,i would rec doing it,,,,but do the guides (2) front and rear and the chain,the tensioner,and lower crank sprocket and cam gears,,,,,if the car is in good shape and youre gonna keep it do it,,,if not,,,risk it and let it go,it may never break,,,but i would do it
