There are tow ways to get to the plugs, You can get to them from the top or from the bottom. There's quite a bit a bit of room in the transmission tunnel, and you can reach up and actually touch the plugs. There is trouble with the "alternator plug" (#1) , Hard to reach it from below, and you may ended up removing the alternator bracket and shoving the alternator back.
From the top end,  removed the windshield wipers/cowl assembly, the intake plenum, remove the bracket from the alternator, easy, and the hardest part is just removing the bracket holding a wire harness from the back of the plenum, the bolts are a little hard to get to due to the a/c lines, but after they were out the plenum removed with ease and I replaced the back plugs(with champion double platinum RN14PMP5 OEM's) and replace the spark plug wires with BWD Selects. They are very good wires. Used anti-seize of course. Mounted the wire harness bracket back, put the plenum(with new gasket), vacuum lines, sensors, and alt.bracket back on then changed out the front ones. Replaced Wipers and your good to go.

Note: if from the bottom  rotate the alternator to get the pass side and from below to get the middle and drivers side. You can use a jack stands and a creeper. Takes about an hour. Yes it's a PITA but, going from below greatly reduces the chance of spinning a bolt head off or breaking something else in the process of getting to the plugs. No special tools needed.   Good luck and if your lazy like i am just change the front ones and have a local shop replace the rear, they have the lift and will only cost an hour shop rate in your area. $65.00/hour average

I don't think this is quite the case with the 3.8L engine in the 2005 Caravan. I just finished changing them myself, and all three rear plugs could be easily replaced without moving any fixture. The alternator on this engine is in the front rather than the rear. Reaching over the engine and doing things by feel rather than sight was no more than a little awkward. Fortunately, all three of the rear plugs did not require much torque to loosen, as getting both hands back there for leverage is a tad more awkward... but not impossible.

SOURCE: How do I change the rear 3 spark plugs on a '94 Grand Caravan?

Yes, these are tough to get at from the top. Put the front on jack stands or ramps, and you can reach them a little easier from the bottom. You just need to reach up where the exhaust pipe comes down, and you can even see what you're doing this way.

SOURCE: How to replace spark rear plugs?

I just changed mine a few months ago. If your arms are long and thin enough, you can actually get under the van and reach up along side the exhaust and just barely reach them. It is not fun, but it beats removing the manifold pieces like everyone says. Change the wires while you are down there.

What is the best way to change the rear spark plugs on a 2005 caravan with a 3.3 engine

I recently did the 6 plugs on our 2005 with the 3.8 engine.

It took an hour to do the rears, and 15 min to do the fronts.

Make sure the engine is cold, and put some no seize on the plugs before you install them.

I took my time and started with the pass side rear by reaching around behind. You can see the rear bank much better than on the 96-2000 models.

Use the factory Champion plugs for best results.

The upper part of your intake manifold needs to be removed.I think Chilton lists spark plug replacement on your Caravan at something like 3 hours.

The P0456 code has nothing to do with spark plugs. It's an EVAP (emissions) code. Best bet is to read up on the suggested solutions for that exact code: P0456

Changing spark plugs

as you know the front ones are rather easy to get to, but the rear bank is a bear..the best way is on a lift but you can get to them using a 3/8 rachet a 3inch extension and the plug socket.. take the air tube off the throttle body.. reach under the throttle body behind the engine you will feel the plug wires that way.. once you get the wires off the plugs are a pain but changable..
