The first thing that i would do would be to give it a complete tune up. then i would run some complete fuel system cleaner through the tank

Should the overdrive be on and does it affect gas mileage?

as a rule, no. many overdrives are automatic and don't engage until you reach 40- 45 mph and they do this by themselves. yes, this does affect gas mileage because the purpose of overdrive is to shift the transmission into a higher gear producing less strain on motor and ****** thereby increasing fuel mileage. conversely, if ****** is in high gear at lower speeds this increases strain (motor feels like it's lugging) reducing mileage. sounds as though you have manually selective overdrive and you will have to monitor mileage both with button pushed and without. then try pushing the button only when you reach 40-45 but you'll have to be continually conscious of the speeds at which you're driving.

F350 2010 superduty vibration at 45 to 50 mph

There are a couple Technical Service Bulletins (TSBs) published by Ford Motor Company concerning front-end/driveline shake and noises. One has to do with the companion flange angle on your drive shafts and the other has to do with contact being made with the jounce bumper on the fron axles (4X4 only)
My recommendation is to take it to a professional auto repair technician and have him drive it and give you his opinion of whether these TSBs may apply to your situation.

sounds like a burnt out catalytic converter. I had this problem with one of my sons caddy's we replaced the CAT and it was fine. But it could be the the fuel injector (s) are clogged. Do you use injector cleaner when you gas up once a month??
