OK. there is 2 temperature switches in the thermostat disconnect 1 and bridge the connector if it shuts off the fan replace the switch do the same with the other one.
the relay's are in the engine compartment on the drivers side this are the first 3 black ones in the slim part of the box

SOURCE: The cover to the fuse box on my 1997 Avalon is missing and that is where the fuse locations are shown. I need the description for the fuses so I will know what they go to. My air conditioner fan will
Try finding a cover at a wrecking yard
or...use a test lite, to check the fuses.
You can find one at the autoparts store.

SOURCE: coolant fan switch
it's hooked up to the temperature sensor. if you don't get a temp light on, the sensor might be bad. it's usually located where the upper radiator hose goes into the engine. also check your thermostat.

The fan also comes on when the AC is engaged. The problem could be in the AC. Try to unplug ac relays to see if the fan will turn off.
