Check wiring to motor and it should read around 12v. (digital multimeter) unplug motor and check resistance at motor. It should read something and if not the motor is dead.
My first thought would be a bad ground connection in the harness for the rear lights. You might want to take a test light and check the contacts in one of the sockets.
First check the headlight relay by changing it with one of the same number in the fuse panel.If it checks out ok then you have a faulty headlight switch.
in the rear lights you may have two bulbs one is the turn signal the other one is stop light / tail light the stop light / tail light has two filaments, one for the stop light (21 watt) & one for the tail light (5 watt) it is possible that the tail light filament is blown, try replacing the bulb to see if that fixes the problem,also check all the fuses as well.......hope this helps......cheers.
From your description, T/S not working with headlamps on, it seems apparent that your turn signals have lost their ground and are grounding back through the headlamps when they are not on.
