There never has been what you want

Only a rough or partial schematic of brake
tubing, is ever in a factory service manual

Only if you intend to work on your own vehicles,
would you get involved in replacing rusted brake
tubing & then you just duplicate what is there, that
needs replacing.

Where can I get a brake line schematic for my 2000 GMC Sierra ? Some lines were replaced and I think there might be a mix up.

Leaking brake line

You need to remove it. It can be replaced in sections using couplers but it should not be put together with ferrule type unions. You may need an adapter fitting on the master cylinder. Make sure everything is secure and cannot vibrate. use as many of the factory mounts as possible but you do not need to follow original routing exactly (this stuff is put in really tight sometimes) be careful bending tubing any kinks will result in a restriction or early failure..

Rear brake line that runs from caliper to frame needs replaced

You can purchase different size (diameter) and length brake lines at an auto parts store. They are inexpensive. The lines are already 'flared' to be connected to another line, all you have to do is disconnect the old line, and attach the new one. Spray the connections several times a day for a couple of days to make them easier to separate. you will also need to bleed air out of the replaced section for the brakes to work properly.
