When i put the car in park and go to drive its very hard to get in gear the truck jerks then when in gear it runs so smooth

sounds like the torque converter lock up might not be disengaging. You might also want to check for engine vacuum leaks. Does engine idle smoothly when in park or nuetral ? if the idle is good I would suspect torque converter problem.

I had a similar problem with my 1997 F250 only with 1st & 2nd gear. At the same time the ABS light was on and the speedometer was acting eratic. In my case it was a bad speed sensor on the rear differential. About $30.00 and took me less than 15 minutes to fix.
I'm assuming you have already looked at the trans for any loose or worn electrical or vacuum lines and connections.

It sounds like your idle air control valve could be going out on you. The extra stress from being in gear can cause the car to stall out if the IAC valve isn't opperating properly. The IAC is attached to the throttle body and is normally held on by two screws. IAC valves are relatively cheap from AutoZone and easy to replace.
