Engine suddenly revs when stopped & @ idling

While stopped in traffic at a light, with my foot on the brake, transmission in 'D' and engine at idle, the engine rpms suddenly increased sharply and the car started forward.  I pushed so hard on the brake pedal that the ABS kicked in, but the car kept moving forward anyway.  By the time I shifted into "N," I had lightly bumped the car ahead - no damage to either car.  I shut off the engine, waited a half minute, restarted and drove home w/o any problems.  I checked for a floor mat jam under the pedal or if my foot could have accidentally been on both pedals at once.  Nope.  I had just exited a freeway and the car had decelerated and braked just fine.  My car has 66,000 mi.  This is the second time this has happened.  The first was around 14,000 mi.  The Mazda dealer found nothing wrong then and it hasn't happened again until now.  Is this problem maybe in the "drive by wire" accelerator control system, the throttle body, or just where?  I'm afraid of causing an accident if this isn't solved.

Sounds like one of the metal arms connected to the engine may be getting stuck some how. have someone get in car and press on gas when car is off while you watch engine to see what is moving, then check those arms to see if there is any possible obstructions or knicks or something on them

Mine is doing this too! When I notice the car slowing faster than normal when I let off the gas and the brake pedal becoming harder and then it revs and pushes forward in drive while coasting I have to turn it off wait a few minutes and it seems fine

test the brake booster
start the car and idle with the handbrake on. If it idles ok then apply the foot brake
if the engine dies or stops --replace the brake booster
If the engine will not ilel without the foot brake check for a vacuum leak in --the transmission vacuum line , vent control lines or manifold leak.

When in stop and go traffic yesterday, the car suddenly died. It restarted, but would not maintain a normal idle unless kept my foot on the gas. Once we got out of the stop and go traffic it ran ok,

It will probably be OK. But if you can, find a can of brake cleaner and a toothbrush. Remove the air plenum from the throttle body. Use the cleaner to clean the throttle body inside and the back of the throttle plate as best you can. Especially around where the throttle plate sits. It will take a second or two to restart the car. Once started spray some of the brake clean into the throttle, rev it up to keep in running. This will help stop the dieing out.

When stopped at light/stop sign, rattling comes

Inspect the motor mounts for cracks or breaks in the mount. Ask a friend to stand at the right front fender with the hood open while you start the car. Make sure the emergency brake is on and you block one of the rear tires in front of it and in back of it with a brick or similar solid material. Put the car in drive (1st gear for manual transmission) and with your foot on the brake pedal rev the motor for a few seconds at about 1500-1800 rpm as your friend watches for excessive left and right movement from the engine. Then do the same in reverse and have him stand on the left side and do the same. If one mount is bad it is recommended that all the engine mounts be replaced. There are usually 2 or 3 for the engine and one for the transmission.
