Question about 1999 Plymouth Voyager

I sounds like it is the heater hose assembly. This vehicle, if I recall correctly, has an assembly of steel pipes that run the coolant into the interior of the vehicle for the HVAC. If it is in the back like you say, I would check those pipes first. If it is more to the front of the car and engine, it may be the water pump.
The best way to check a cooling system is to pump pressure into the system and check for leaks.
