No other symptoms.?
is it the engine that hesitates or the slushpump A/T tranny.

RPM of engine tells you that.
goes way high? that is slip, bad tranny
stay about 2000RPM max with brakes held.?
fail; hot or cold engines.
or is this post, that when you move hand from N to D there is a delay
you never said what is delaying , car movement or shifter to trans engagement.? or ?

1990 Chevy Van with a 350 small block that has a mind of it's own.

Try to get it running a bit to warm up the exhaust. Take a point and shoot thermometer and take the temp of the cat. If temps exceed 400 degrees I would start to suspect a plugged cat.

If my memory serves me right your van has a carburator. The stumble is in there. Take it to a reputable shop that specializes in rebuilding carbs and fuel systems. Time has taken its toll on yours but a good cleaning and rebuild should cure your stumble. Hope this helps.

Transmission still shifts erractically

well if you have a lot of millage it could only be the gearpump in the transy thats finished, unfotuntly ...you cant really change -it without rebuilding the all transy
other than that you still can drive-it but try letting the gear go on by it self ,you could damage your van by doing that !! you mignt not have choice but if you do ...try not doing-it!
