The first d is over drive . This helps with take offs ,slowing down,
going up hills or towing . The second d is normal driving . I
keep mine in over drive all the time . It dont really use much more gas either

It's a normal engine sound, but a lot louder. I can hear it inside and outside the car. If I put it in reverse or drive it smooth out and is normal. If I put it in neutral and it's loud too. When I drive,...

It may be just the engine. If you notice that the rpm is not constant you may need to have the IAC cleaned out or the vacuum lines checked for leaks or loose connections. It could also be leaks or holes in the exhaust pipe. When running road noise may have canceled it out.

Was driving at45 mph when it sounded like it went into neutral so i put in third and it caught but i dont have drive is the transmission bad or could it be repaired fairly cheap

First thing you need to try is to check the transmission fluid level,do this when the car is up to normal temperature, put it in drive the reverse a few time then return it to park.Pull out the dip stick[drivers side just below the battery],wipe it of and put it back in,pull it out and see if it's full.If it's down at all,top it up with the proper transmission fluid and it should work normally,if not have a transmission shop look at it.

Car jerks when put into drive

check the engine/trans mounts. A good way to check yourself is to have someone standing in front of the engine with the hood open obviously, while you shift the tranny to drive or reverse, if your friends see's the engine moving vigorously, it's worn out mounts.
