BI I had the same problem and the only solution was in changing the belt to one that had the standard long all the way around the belt grooves. All of the other type allowed water under the surface and since water does not compress it reduced the traction area of the the belt and the belt slid off off the pully.

Have you changed the tensioner assembly as it wears it will tip and let the belt walk off the pully also there is a drain tube on the bottom of the wiper tray if that is mising or broke all the water from the tray drains onto the belt either will cause the belt to slip off let me know how youmake out
