If fluid isn't burned, shifts aren't slipping or flairing and you did not find a large amount of debris in the pan, chances are that the electronic shift control is malfunctioning. Have someone check it out.

2003 toyota echo when shifting above 45 mph makes a loud high rpm noise especially on the highway

If it is not going into overdrive or lockup it could be the fluid or an electrical problem. Some of the shifting is controlled by solenoids now.
You may have to get a transmission shop to evaluate it for you.

On a 2000 ram 1500 rear wheel drive above 45 mph the od doesn't work

Yes an electrical problem should set a trouble code in the engine or body computer.
It could be a pressure problem inside the transmission.

97 aurora shifts fine in 1st thru 3rd and under 45 mph, then over that speed will go into OD but slips over 45 mph, any ideas??

If the AFT fluid level is ok then the only thing that would cause this is that the transmission has failed, the clutches are burnt up or worn out, the transmission will need to be replaced or rebuilt.

How do i know if the transmission is going out?

Looking at the fuild is usually a good start, if it's not real red there could be a problem. Also the fluid having a burnt smell to it. Also, when driven, how does it shift? Does it slip into gear, or shift crisply? It should shift into overdrive around 45-50 mph.
