Map sensor bad or vacuum &/or power to it missing. Map sensor located on intake manifold or firewall. It has a vacuum hose & electrical connector attached to it. To verify it is bad disconnect electrical connector to it while running. Idle should smooth out & run good. If so need to replace.

The sensors are dirty and covered in soot or the EGR is dirty or clogged and overheats. Clean them all then make sure the sensors are in good working order. If the problem returns you will have to replace them.
