Check the heat shield on the Catalytic Converter, after a few years they start to rust off and rattle. Crawl under the Pathfinder and bang along the Converter and muffler area and see where it's coming from.   Good chances are, it's from the heat shield since you already had everything changed from the Cat back.

I have a rattle noise going over bumps. I sounds as though it is coming from drivers side rear. No noise going over smooth roads. It sound as though something is loose

Usual problem is tailpipe/muffler. Clamp attaching to body failed, so muffler will hit the body making noise. worse is rusted out muffler/tailpipe.

When I first start my truck I can hear a loud rattling sound coming from the back. Hear this sound also when I am in idle. Don't notice it when I pick up speed.

Check to see if one of your brackets holding the exhaust pipe has come loose. That'll make a rattling sound - especially if it towards the end by the tailpipe. When you're accelerating - more air is forced thru your exhaust at a faster rate, so it's not moving as much as when you're sitting still and the truck is just at an idle with less air/exhaust moving thru the tailpipe. If it's only making the noise while the truck is stationary & the noise stops one the truck is moving - that pretty much rules out anything to do withe the drive-train or suspension. You are sure the noise is from the back & not coming from under the hood? If under the hood - check your oil level ASAP.

Rattling sound from the exhaust

The metal shielding around your catalytic converter could be rattling or the rubber donut that holds the rear tailpipe could be bad. When your car is cold shake your tailpipe and see if you can reproduce the noise. If you can your tailpipe donut is likely bad. Three dollars at any auto parts store. If it does rattle when you shake your tailpipe then the heat shield for your convert could be loose. You need to tighten it up another piece of metal between it and the converter to keep it from rattling. If you hear the rattle when you go over bumps too then your exhuast is probably, loose. Crawl under there and stake shaking things, I am sure you'll find the problem

My car makes a rattling sound under the car most notably when im moving and i rev the engine hard and put it into gear. It has gotten worse over time, i think it might have something to do with muffler...

Yep, there's a good chance some part of the exhaust system has come loose and is rattling at certain RPM's. Start fault-tracing by checking if any part of the exhaust seems loose to come in contact with anything, or if there's any loose clamps or similar parts hanging onto it. Another quite common reason for rattle is that some part of the exhaust manifold, such as a bolt or hose/pipe has loosened (often due to rust and/or corrosion).
Another possibility is that it's somehow related to the transmission (since you mention putting it into gear). If so that would be another subject.

Muffler Rattling

Sounds like a broken U clamp. Get under the car and pull on the exhaust (while its cold) and see where the tailpipe has broken free from the bottom of the car. Then secure it with a new clamp. In a pinch, have always used a wire hanger for emergency repair.

Muffler is loud and rattles due to debris and possible hole

either live with it or get out a saw and cut it out and weld in a new exhaust muffler. or you might get the measurements and get one to fit where you cut out the old one from your parts store.. make sure you measure your pipe and get a muffler that will fit over the pipe .. You will need clamps for the muffler also.. thanks and holler if you need more Jerry

04 Endeavor rattling noise

I had the same problem! I crawled underneath the car and discovered that a heat shield on the exhaust header pipe was corroded and rattling against the exhaust pipe. (Before the muffler)
I removed the portion and clamps and solved the problem. Give it a try.
