Why does the transmission on my 91 accord get stuck at 45 mph and rev at high rpms

That gear is damaged in transmission. The car gets stuck because of that. The gears lock together. The car Rev because transmission won't move and engine power keeps increasing.

The engine revs and rpm go up but wont go faster then 40mph

First thing you should do is check your transmission fluid level. This is a common issue when it comes to high rev, and low acceleration. If its a manual tranny, the clutch could be slipping, is their a burning odor coming from your car?

Driving at 40 mph and rpms were at 6000

Well with no other info, your transmission might not be shifting. Please check fluid level and reply. Also is the check, engine light on? You may be on limp mode if it is on. Worst case you need a new transmission best case low fluid level.

96 escort 1.9 manual jerking while accelerating 96 escort 1.9 manual

best advice is dont go 40mph in 5th gear... if you are going up a hill at 40mph in 5th gear i have no doubt that it is jerking, your putting too much stress on the engine. shift up to 4th or 3rd you will be fine. 5th gear is an overdrive gear. 55+mph

My 2001 ford escape suv doesn't go more than 40 miles p.h. when i mean to go faster

Is the engine revving high at 40mph? If so then chances are the trans has not shifted. Is engine very sluggish, or won't rev high, possibly with a hissing sound? If so then you may have a restricted exhaust system.

Transmission is not shifting properly on my 2000 nissan frontier 2wd pick up, auto It revs at 3-4000 rpm instead of a nice consitent 2000rpm, revs real high and doesnt change gears intill 20 mph, it doesnt...

You could have a bad valvebody in your transmission or a bad pump in your transmission or your clutches are bad. In any case you have a bad transmission. Please rate my answer.
