What's the problem when I hit the gas it has a miss in it especially when I try to pass a vehicle, it's almost like it could be a exhaust leak or a leak in the the vacuum line?

Exhaust leak won't cause a miss, vacuum leak can cause bad idle, but any of several other things could cause your miss, including:
Bad sparkplug
Bad plug wire
Bad injector
and, more serious issues like
Bad valve
Head gasket

Engine misses when going up incline until I let off gas

Missing under load can be caused by a number of different things including:
a bad sparkplug
bad ignition wires
if your car is old enough,
bad distributor cap

Also, it's possible it's missing all the time, and you only notice it under load. If so, then add
bad valve
bad piston or rings
blown head gasket

Missing fire

a bad injector can cause a miss, you can listen to each injector with a stethoscope. It should make a rapid clicking sound. If it makes a deep thumping sound it is likely bad. also an intake leak can cause a miss at idle. also an EGR valve partially open can cause miss at idle. I always check cylinder compression when replacing plugs or coils for a miss. You need quality tech to check for above items.

1995 Jetta: vacumn hoses missing causing car to miss...

A missing vacuum line will not cause a car to misfire, it will run rough because of un-metered air entering the intake system. A misfire could be from bad spark plugs and wires. A malfunctioning ignition system, a bad fuel injector or fuel pressure charging component. An end result for not correcting a misfire problem is a damaged catalytic converter. From the factory it should come with a vacuum routing label usually on top of the radiator carrier or hood locking system. It will tell you specific location for the vacuum lines.

You might have a bad ignition coil. Exchange the #5 coil with another position and see if the "missing" follows the suspected bad coil. If not, then check for a bad # 5 spark plug wire.
