Not sure, but 5th may be a lock up torque converter rather than another "real" gear. If so, solenoid for this may be bad, or torque converter may be bad. Have you changed the trans fluid and filter?

either the synchronizer in you transmission is bad or possibly the car has spark plug misfire make sure the sparkplug wires are in the proper order

I hate to say it but given the mileage on the car the transaxle may be going out. Usually the symptom is caused by problems with the syncro gears but only one or two. For example, if it only jumps out of 5th gear it would be the 5th gear syncro.
You could check the adjustment of the cable from the shifter to eliminate that possibility.

Gears change without a command from the driver.


There is a vacuum controlled selector valve on the selespeed alfa's it tricky to get at and expensive to replace. Have it sent to an alfa specialist or main dealer and they ll sort it

On occasions when in 5th gear my car jumps out of gear into neutral, it has just started doing this.


this is caused by internal wear in the trans, this car must have hi miles, u will need to have the trans removed and repaired.
