It is a vacuum hose that is disconnected. if you can find the hoses under the dash you can see what is disconnected. You can also purchase a Haynes Manual and that will help diagnose any problem, or spend a little $ and have a pro look at it. It wouldn't be that expensive, since it's a simple fix.

Why do condensation and frost come from air vents and can this be related to windows fogging unrelentingly in winter?

Suggest you check to see if your car has a cabin air filter. If so, changing this usually helps prevent the issue.

Where is the fuel pump relay located at?

Airconditioning blowing through the de-frost vents but not through the driver and passenger vents.

that means it is in the 'default' mode, so you have defrost capabilities-check first for a disconnected, very thin vacuum line-it will pass through the firewall on the passenger side to the top of the engine and may be disconnected, or have a crack or leak. If you find it and cannot see anything wrong with it, the next thing is usually the controller itself, which will require removing the surround panel around it and removing the screws that hold it into the dash. Check the harness and connector for loose fitting or possible damage. If you cannot find any obvious problem, chances are good that the controller itself is bad-the can usually be found on ebay for less than $50 including shipping. The most common issue is the vacuum line, so look carefully there first.

Ice formed on outside of line!

try running the air conditioner in defrost mode. set the temperature to warmest while air conditioner is on, for about two minutes. then slowly move the lever towards cold, all the while having the vent set on outside air. once you move it toward cooler, move the vent switch towards recirculate from outside air.

when you have it on defrost, open the engine hood and push the air conditioner button. watch the lines and see if they begin to build frost. if not, move the slider toward cold a little bit at a time while watching the lines to see if they begin to build frost. if they do start to build frost at any point after running the defroster in A/C mode for a bit, you can always try positioning the temperature control towards the middle or run only the outside instead of recirculating vent.
