This could be a lot of things, CV joint, tie rods, wheel bearings, even loose lug nuts.

If you put new rotors you need new brakes as well the other thing to check is the inner and outer tie rods to do that get the front tires off the ground grab the front and back of the tire on the tread have someone look at the inner and outer tie rods as you move the tire back and forth if the tie rod does not move it is wore out you can also look at the sway bar pins and make sure that they are not broken

Why does my 1998 plymouth breeze sound like my right tire is going to fall off when i turn the wheel to the right? and its only the front drivers side!

You could have a bad wheel bearing/hub assembly, loose tie rod (inner or outer), or a loose ball joint. If you raise the vehicle (with lift or jack), shake the wheel left and right and if it seems to have any loose motion not controlled by the steering wheel, it will be one of those i just listed. Shaking top to bottom is usually an indicator of a bad wheel bearing while side to side will be one of the others.

Mechanic says inner tie rod needs to be replaced. How do I know?

Raise the vehicle and wiggle the wheels left and right. You would know which one has a play if its the inner or the outer. the inner tie rod connects close to the rack and pinion. The outer is close to the tire. Goodluck

How do I replace just the outer tie rod(passenger side) of a 2000 plymouth neon? I have the tie rod, what equipment will I need?

you can do the whole job with a large crescent wrench, hammer, and pliers. Loosen the lock nut but only loosen, remove the cotter pin (if present) and joint nut. Hit joint with hammer to separate from steering knuckle. unscrew tie rod end. Compare length to new end. You will want to keep the length from the nut to the center of the hub arm attach the same or you will need an alignment. Screw the new end on and adjust to the original length. Slip back into hub, replace and tighten net, repace cotter pin. Tighten lock nut and get an alignment if you aren't sure about length. OK? Any questions, let me know.

Im trying to find out what the proper tire rod part is for a 1987 plymouth reliant for the front right wheel

The local parts store should be able to look up the proper tie rod for you. You will need to determine if it is the outer or inner, or both by checking for slack or excessive movement.
