There is a temperature sensor in the engine block. This controls the fan. Sometmes it can be part of the thermostat but most usually it is a small switch that looks like a bolt with only one lead coming from it. It is possible to buy these temp sensors to retro fit into the cooling pipes. They can then be connected to the fan relay. They usually switch an earth for the relay coil, this will allow the power to the fan. They usually switch at 80 to 90 deg C.- This is the optimum temperature for an internal combustion engine.

SOURCE: 02 PT Radiator Fan Stops Working Intermittently - Car Overheats

I had the same problem and researched it and found 1000's of other people having the same problem and spending lots of money replacing water pumps, radiators, thermostats, switches, relays,,, etc. What I did was check the cooling fan with 12 volts and found 1 speed doesn't work. I wired the fan so it comes on when the key is turned on and goes off when you turn the key off. The a/c is cooler than it has been in 3-4 years. The temp guage goes to right under half and it doesnt matter if you sit and idle or drive the interstate the temp stays the same and the a/c will run you out of the car. Fixed for less than $5.00.

SOURCE: radiator fan not working replaced relay, temp

You didn’t mention replacing the temperature sensor located on the block in the path of the ‘cooled’ coolant.
I can’t tell you any closer where to look for it since you didn’t specify which engine you have.
This device is a solid state item called a thermistor.
With the air temperature around 77 degrees F and a 'cold' engine, it should measure about 10,000 Ohms and hot, around 1,200 Ohms while disconnected from its mating connector.
With the ignition on, and the connector disconnected from the sensor, you should measure about 5 volts coming from the ECU.
If this has failed, the ECU is not getting the right info back from the sensor and doesn’t know if the fan is needed or not.

Fans wont shut off when ignition is off 99 prelude

Some cars are designed to allow the fans to run even with the key off until the engine and radiator cool down. If it continues to run after 19-15 minutes you may have a sticking fan relay, usually located in the fuse box in the engine compartment, just replace it and it should take care of your problem

You have to check the main relay fan fuse relay and disconnect the fan and try it directly to make sure the fan motor is fine if all this is well into the cavity where the thermostat is a thermal switch that sends it relay the signal to the fan you can disconnect and the fan has to start working if this works the problem is you have the thermal switch.
