The PO401 you can Google
It is an EGR Issue
The fuel trim could be vacuum leaks, oxygen sensor
issue,cylinder ignition misfires.
Once you resolve the Egr System problem, you continue on
with your testing. Insufficient flow means you have less exhaust
gas re-entering the intake, than you should have,so I would say
you have two or more issues

SOURCE: emission test gives a p0401 code : exhaust gas

yes it is something to do with the egr
it could be the egr, dpfe, vacuum leak or wiring
best bet is to bring it to the dealer to have it properly diagnosed
so your not changing unnecessary parts

I need to know what the problem the p0401 what is the problem 1999 grand Victoria Ford 4.9

wow I wish ford could make a egr system that wouldn't fail as I've seen this one go a lot lately ! anyways the code is for insufficient flow in the egr system , this is a means to recirculate some of your exhaust back into your intake , as a means to reburn some of your excess fuel (emissions) , anyways it consists of a means to meter some of the exhaust , and control it (off/on) also to measure the amount that goes in , its part of the closed loop of the fuel inlet system , the fault code is specifically saying flow sensor cant detect any flow in the pipe , there can be a few causes for this including a bad flow sensor , all are from my opinion a poorly designed system (it doesn't self clean it's self ) as almost a guaranty every 60k miles your gonna have some problem with it

You could have leaking fuel injectors an fouled spark plugs causing the misfire ! I would pull the spark plugs out an see if wet with fuel . Could also have a leaking fuel pressure regulator ! How To Fix P0300 random misfire codes in your car

O2 Sensor Signal Fixed Rich http://youtu.be/H6lY42u19VM
The front oxygen sensor sends signals on exhaust gas oxygen content to the PCM to control a 14.7:1 air/fuel ratio under normal driving conditions. The PCM can make fuel corrections from a nominal 128 (0% correction) short/long term fuel trim value to a maximum window of 101-147 (-21 to +15 percent). If the PCM determines a rich condition exists, 02 voltage above 450 mV, it will decrease injector pulse width, reducing fuel, to maintain a 14.7:1 air/fuel ratio. The short term fuel trim value will be between 101-128 indicating a rich condition. If the PCM determines a lean condition exists, 02 voltage below 450 mV, it will increase injector pulse width, adding fuel, maintaining a correct air/fuel mixture. The short term fuel trim value will be between 128-147 indicating a lean condition. When certain conditions have been met to begin learning fuel control (ECT, closed loop, etc.) the PCM will use the short term fuel trim cell to update 1 of the 4 long term cells (idle, decel, cruise, accel) being used. During every drive cycle, or trip, the PCM will use and update the idle and cruise long term cells and will keep track of how long the control system is operating in these cells. After approximately 2 minutes of being in each of these cells, the fuel control system will enable EVAP purge and the purge long term cells will be used. Refer to EVAP Canister Purge for information on purge long term fuel cells. If the average of the idle and cruise fuel cells is 147 and the short term fuel trim cell is currently over 128, a lean fuel system DTC will set. This test is performed before the purge cells are used.

96 acura tl 2.5 liter, short term fuel trim at negative 20 percent

Do a cylinder leak down test, for bad valves on #3
or head gasket, misfires for a reason

(Cam Worn ?)

Have to get rid of any misfires

If your taking out 20% your correcting for a rich condition, maybe

Some injector issues, old, need to be sent away for cleaning & flow
testing or replaced?

Drive it for 30 miles & add short & long term & see what you get

Exhaust Converter any good ?

Hi there i have 1997 Avalon V6 . the check engine light up # 138 and 401 so can you tell me the name of that two please thnks

Trouble codes are normaly 4 digits with a "P" or "B" before it. I am gonna assume that you meant P0138 and P0401:
P0138 is Oxygen Sensor Circuit High Voltage (Bank 1, Sensor 2)
P0401 is Exhaust Gas Recirculation Flow Insufficient Detected

A code P0138 may mean that one or more of the following has happened:
  • Faulty O2 sensor
  • Short to battery voltage in O2 sensor signal circuit
  • High fuel pressure (not as likely)
Possible Solutions Here are some potential solutions:
  • Replace O2 sensor
  • Repair short to battery voltage in o2 sensor signal circuit
for info on the P0401:

What does trouble code P0171 for a 2002 Buick Park Avenue mean?

Trouble Code: P0171 (3.8L V6 VIN 1 Auto)
Fuel Trim System Lean (Bank 1) Print this code dataprint_icon_codes.gif Number of Trips to Set Code: 2
OBD II Monitor Type: CCM Details
Indicators: MIL Details
Trouble Code Conditions:
DTC P0101-P0103, P0107, P0108, P0121-P0123, P0130-P0138, P0140, P0141, P0201-P0206, P0300, P0401-P0405, P0410, P0440-P0446, P0506, P0507, P1404 or P1441 not set, engine started, vehicle driven at less than 82 mph at 550-4000 rpm, BARO sensor over 70 kPa (10.1 psi), ECT sensor from 68-230ºF, IAT sensor from 64-158ºF, MAF sensor from 2.8-150 gm/s, MAP sensor from 16-105 kPa (2.6-15.2 psi), fuel level over 10%, and the PCM detected the Long Term fuel trim value was more than +20% for 6 seconds. The PCM controls the air/fuel metering system to good overall fuel economy, driveability, and emission control. During open loop, the PCM determines fuel delivery based on sensor signals, without the oxygen sensor input. During closed loop, the PCM adds oxygen sensor inputs to calculate Short and Long term fuel trim fuel delivery adjustments. Short Term fuel trim values change rapidly in response to changes in the oxygen sensor signals. Long Term fuel trim makes coarse adjustments in order to maintain an air/fuel ratio of 14.7:1.

Possible Causes:
  • Air leaks in intake manifold, exhaust pipes or exhaust manifold
  • Fuel control sensor is out of calibration (ECT, IAT or MAF)
  • Low fuel pressure (fuel filter clogged, pressure regulator failure)
  • One or more injectors restricted or pressure regulator has failed
  • HO2S element is contaminated, deteriorated or has failed
  • Vacuum hose is disconnected, broken, leaking or loose

I'm getting an engine check light, the OBD-II is P0401.

try to clean out the tubes, it is very common for the tubes to get clogged up with carbon, also check for any breaks in the vacuum lines
