Question about 1996 Ford Explorer
The computer uses data from the front and rear O2 sensor to monitor catalytic converter activity. The sensors are probably telling the computer the cat is not functioning correctly.
The problem could be the cat, or something else including the fuel system or cooling system.

OK. The Code bank 1 or 2, have to be acompained with a especific code number.
Because Bank 1 or 2, is Left side or Right side.
On windstar Bank 1 is at the firewall side, and Bank 2 is at the radiator side.
But the number of the Code will give the especific problem on any of the Banks. After that you gonna know what you have to fix.

Replace the O2 sensors . they have one in the front and one in the back of the Catalytic Coverter pipes.Scan the SUV again to see if the codes were cleared.Meaning no "check engine light" on.

many o-2 sensors go bad after about 60,000 miles they will cost you gas mileage,they tell computer engine is lean and causes more fuel to be dumped in when not needed--go to autozone if it is 96 and above they will scan it free give u possible causes and explanations! if sensor indicates lean check the fuel trim if long term fuel trim is above +7 them test sensor with a piko lab scope or watch scan data and create a rich condtion by snping the throttle wide open and watch the sensor if it start to read around 700to 900mv its ok, then create a vacume leak and the sensor should read 100to 300mv i your sensor is in this range quickly its good with in 1to2 sec.

The code Im assuming is po420, or po430. In either case it means the catalytic converter is inert and no longer has the ability to convert hydrocarbons to CO.
