Is that the clutch or the transmission? If it's the transmission, should I replace the clutch as well? Vice versa?

sounds like a syncronizer. 99 model, probably no warranty, not good. Trans is difficult to dissasemble, so i would have to tell you to leave that one up to the pros or find a used one to swap out. Don't hesitate to get a second opinion though.

SOURCE: Transmission hesitates shifting into second and perhaps third gear when driving slow. When acceleration is positive transmission shifts smoothly. Reference "99 Mercury Transmission" as Subject when
Several things could be causing your problem:
p.s. If you have the fluid changed, make sure it's done from the tranmission pan, not through the filler tube; siphoning through the filler tube can be hazardous to the transmission, especially cars with a lot of miles

This is being caused by the seals in the transmission worn out and the heat is causing them to become to plyable thus losing pressure, an overhual is needed

Have you checked the end float on the cranK, try pulling and pushing the pulley on the crank, those engines were famous for wearing the thrust washers and then they drop out they take the "push" from the clutch release mechanism and if the play is too big it causes drag.
