if you have a obd11 scanner, clear the codes and drive the vehicule for a while ,a good half an hour or so above 30 mph. this will give your onboard computer a chance to relearn system settings.if the code comes back there could be some other sensor malfunctioning or a problem with the onboard computer.ex corroded connections or broken pins.
P1491 - Radiator Fan Control Relay Circuit Open Or Shorted
When monitored: with the ignition key on, battery voltage greater than 10.0 volts and the pcm requests the radiator fan to be turned on. Set condition: an open or shorted circuit is detected in the radiator fan relay control circuit.

This code indicates their is a fault with in the radiator cooling fan circuit. are their any other symptoms or just this code showing? it may be that it something electronic has failed, and the thermostatic fan will just run at full speed, instead of cutting in and out as required to regulate temperature. If fan is runing, their is no harm in driving vehicle.
P1491 is Radiator Fan Control Relay Circuit. MEANING: An open or shorted condition detected in the low speed radiator fan control circuit.

2. Is your Caravan water temperature spiking going past the midway point on the Temp Gauge before the radiator is engaged?
3. Is your Rad Fan coming-on and then turning off and back-on in a very short period of time (like under a minute?
