Question about 1992 Ford Ranger
When I accelerate for a long time, on a highway for example. I start to hear a high pitch noise. It doesn't sound super bad. It starts out quite and gets louder. If i stop accelerating it stops immediately, but starts doing it again after a bit. I also have a second problem. Rattling in my gear box when i go over bumpy roads. It's super annoying, The noise is coming right from under my stick.
That could be a loose belt. Sometimes it is caused by a vacuum leak. It could also be a transmission sound. Check the transmission oil level. That sometimes causes a whining sound.

Sound like main shaft bearing assuming it is manual( if it comes on all the gears)
if it is automatic it must be your torque converter bearing

First thing i would try...... ISOHEET, Red bottle..... allows water to be mixed into the fuel and be burnt out of the tank.
get a good qaulity fuel injection cleaner, if you can locate the brand MALCO, i found it quite succesful..... and go with a full tank of premium fuel such as ammoco or shell......do not use yellow bottle of heet
