A few things that could cause the problem are:1 Airflow meter that is if it is a fuel injected motor. 2 On carburettor models the idle solenoid will do that if it is faulty: 3 Airconditioning compressor or pump will click in and out with a sound causing the revs of the engine to alternate as it uses more or less energy accordingly. Also if the bearings in the compressor are worn in it's early stages of wear could make a similar sound to that of a fan: 4 The electric fan coming on too often because the engine is running very hot, could also cause the clicking sound as the thermostat clicks on and off. I suggest you check the water level in the bottle and in the radiator just to make sure that it's not getting too hot and if the levels are low then you have a problem that must be attended to immediately.

Check the vacuum hoses, but sounds like the IAC or even the o2 sensors. The computer will pick up on the o2 sensors but not the IAC..Go to your local ADVANCE store and they will check on computer for free :)
SOURCE: 2001 v6 escape rough idle when cold
To update my earlier repair on the 2001 6 cyl Escape: When cold, and when the ignition was turned over but no accelerator pressure given, the rough idle finally returned the P0303 code again when the check engine light came on indicating that it wasn't caused by the plugs or the coil. FYI, I had replaced the intake manifold gaskets during the original repair and just prior to that I had replaced the fuel filter which was very dirty (I cut it open to see).
The repair manual referred to fuel injectors as a possible problem. I have run three tanks of fuel using different injector cleaning products and am very pleased with the results. Cold starts are much smoother and I am unable to coax out the P0303 code or any other code.
My opinion, change the fuel filter, use a fuel injector cleaner for a couple of tanks, change the plugs if the problem still exists. The coils were not the problem on my vehicle.

If your car is fuel injection, try changing the cold start temp sender.
SOURCE: 88 ford ranger 2.9 low oil pressure
i just bought a BII with famous tick..........oil pressure light came on (no gauge). I installed mechanic gauge and showed 5 psi so I decided to replace oil pump. Pulled pan off (not fun!) the pickup tube was clogged and the was part of the timing chain tensioner in pan. I then pulled timing cover off only to find chain streched and the chain guide and tensioner pretty much gone. I think tick was chain slap..........will know for sure in a couple days.

sorry but you have tried everything i would have done except that i would have checked the engine breather pipes for deterioation like perished or soft and collapsing .you would be surprised how often this is the main cause of this sort of problem
